Dupuytren's contracture is a condition where fibrous tissue in the palm gradually thickens and forms cords that pull one or more fingers into a bent position. It typically starts with small nodules — firm lumps in the palm — and progresses slowly over months or years. The ring and little fingers are most often affected, eventually making it difficult to flatten the hand or extend the fingers.
Dupuytren's is more common in men of Northern European descent, tends to run in families, and becomes more prevalent with age. Diabetes and alcohol use are associated risk factors. The condition itself can be slow to progress — some people live with nodules for years without meaningful contracture. Others progress to significant finger flexion that interferes with daily life: gripping a steering wheel, wearing gloves, washing hands, or shaking hands comfortably. When it gets to that point, intervention — usually needle aponeurotomy or a fasciectomy — is typically recommended.
We most commonly see Dupuytren's patients after a procedure — either a needle aponeurotomy (NA) where the cord is divided with a needle in a clinic, or a surgical fasciectomy where the cord and tissue are removed. Both procedures work well, but neither is a cure — Dupuytren's can recur, and what happens in therapy after the procedure significantly affects both the quality of the result and the durability of it. We also see patients earlier in the process for monitoring, splinting, and education about how to slow progression.
